"These six-monthly reports keep the community up-to-date with incidents occurring in the NSW health system and what is being done about them. They are produced by the Clinical Excellence Commission and the Quality and Safety Branch of the NSW Department of Health. The word "incident" refers to any event that either causes, or has the potential to cause patient harm. These reports are part of the NSW Patient Safety and Clinical Quality Program established in 2004. This program builds on previous systems, structures and knowledge to place patient safety at the centre of all healthcare delivery.
